Eligibility Criteria

1. Current active (non-trainee) member of SATA.
2. A letter of support from a senior level clinician or administrator.
3. A minimum of 5 years of leadership or management experience
   (examples include service, division or operating room director; other
   relevant administrative/leadership roles on the department, hospital
   or system level):
   • a. Personal statement describing prior experience and
     qualifications – (500-word limit).
   • b. Current CV
4. Attendance of at least 1 Annual SATA National Meeting within the
   past 3 years.
5. The applicant holds an MD/DO or an equivalent degree.
6. Fees:
   • a. Application fee of $100 (non-refundable) payable upon
     application submission.
   • b. Certification fee of $150 payable upon approval of the
     application.
